Supporting children with severe autism in their homes and communities
Eligible children are those who are diagnosed with Autism Spectrum Disorder or Pervasive Developmental Disorder, aged one through the end of the semester that the child turns 21 years old. The Coordinating Center provides service coordination in Dorchester and Worcester Counties, as well as Harford and Howard Counties.
Also, children must:
Have an Individualized Educational Program (IEP) or Individualized Family Services Plan (IFSP) and
Receive more than 12 hours per week of special education and related services
Meet an institutional level of care
Not enrolled in any other Medicaid Waiver program (Children can be simultaneously enrolled in REM.)
Eligible children can receive the following services:
Respite Care
Environmental Accessibility Adaptations
Family Training
Supported Employment
Intensive Individual Support Services
Therapeutic Integration Services, such as in an after school program
Residential Habilitation (regular/intensive)
Targeted Case Management
As with the Model Waiver, a child’s fnancial eligibility is based upon the child’s income and resources.
