The Rare and Expensive Care Management Program…addressing the complex medical needs of people in the community
The REM program is part of Maryland’s HealthChoice Medicaid managed care care system. It provides a spectrum of services for people of all ages whose health care needs exceed the scope of services provided by managed care organizations. There are some defined diagnoses that include projected age limitations and people may age out of the program. This statewide program serves inidividuals who are Medicaid beneficiaries and eligible to receive all Medicaid covered services as well as an array of optional medically necessay services, such as in home nursing. Optional services must be preauthorized. One of the REM services is Care Coordination, which is provided by The Coordinating Center statewide. The Center is one of several care coordination agencies which consumers may select.
The Coordinating Center care managers assist families and individuals to navigate systems and coordinate such services as
Doctor’s visits
Inpatient hospital services
Pharmacy
Home health
Rediology and laboratory services
Durable medical equipment
Disposable medical supplies
Therapies and assistive technology
Anyone may refer an indivdiual for the program. Referrals are accepted through the REM Unit at the Maryland Department of Health and Mental Hygiene, 1-800-565-8190.
