Primarily a care management organization, The Center provides this service for individuals and family members who are participating in programs managed through this organization. For the most part, care management and care coordination is funded through Medicaid programs including the programs that have made Maryland a leader in home and community based services. These programs include:
The Model Waiver for children with complex medical needs and disabilities who often required technology and nursing assistance in the home; statewide
The Rare and Expensive Case Management program (REM) for children and adults who have Medicaid eligibiltiy and require a complement of services to thrive at home; statewide
The Living at Home Waiver Program is designed to work with younger adults with comprehensive planning as they transition from nursing homes to homes of their own; statewide
Infants and Toddlers service coordination for childre who need early intervention in specified jurisdictions
Autism Waiver service coordination for children with severe autism that threatens their capacity to remain at home, in specified counties
The Coordinating Center is recognized for technical assistance and consultation on issues related to disabilities and specialized health needs. For example, the organization’s ACCESSGroup is currently implementing Opting for Independence, which is a pilot project for aging in place.
In concert with the Housing Office, The Center is part of two national programs to provide training on transition strategies among states who are participating in the Money Follows the Person movement. (Money Follows the Person provides a mechanism for people receiving public funds for institutional or facility settings to use that funding for home and community settings instead.)
ACCESS Group joins technology and quality principles to assist community providers to improve their own programs and services.
Medical Legal Services provides life care planning services in several regions; life care plans are person centered, meaning that they are written from a neutral perspectie based on knowledge of the person’s needs over time. Expert testimony, long term care management and geriatric care management are also components of this service.
