The Coordinating Center is using the mobile technology, Care at Hand, to reduce hospital readmission rates. The Center was featured in an article “Giving Home Health Care Workers the Power of Data” in USA Today on March 5, 2016.

The Coordinating Center is making a difference in West Baltimore, as it works to reduce hospital readmission rates. The Center was featured in the segment “Baltimore Hospitals Work to Repair Frayed Trust in Black Communities” on PBS NewsHour on February 15, 2016.

An article in the Capital Gazette on November 18 mentions The Coordinating Center’s partnership with Anne Arundel Medical Center to help reduce hospital readmission rates.

On May 6, 2013, The Coordinating Center fulfilled its life long dream of having a new Central Maryland headquarters. The new building will allow The Center to better support the people we serve through expanded services and more efficient use of resources. Expanded services are focused particularly on older adults who wish to stay in their own homes as they age, and on families of young children with complex needs struggling to provide their children with the best opportunities in these changing and challenging times.
