Article about Care Transition Programs Published in Healthcare Executive Magazine

“Replacing a Revolving Door,” an article authored by Carol Marsiglia, Senior Vice President, Strategic Initiatives & Partnerships, The Coordinating Center, Randy R. Komenski, FACHE, Director Clinical Transformation and Clinical Business Operations and Safety Officer for Bon Secours Baltimore Health System and Kim Keaton, Senior Program Manager, Government Affairs and Innovation, Corporation for Supportive Housing, is published in the March/April issue of Healthcare Executive magazine. Healthcare Executive is a publication of the American College of Healthcare Executives.

The article discusses care transition programs, including “Housing First” models that address the needs of  “super users” of hospital Emergency Departments (ED). Super users frequently use the ED for their chronic health care needs and often need access to a stable living environment. Intensive and comprehensive care coordination and supportive housing has proven to decrease the use of public services and costs among this population while also stabilizing their health and housing.
