The Coordinating Center Granted URAC Case Management Accreditation

The Coordinating Center has been granted full Case Management Accreditation Version 5.0 from URAC, a Washington, DC-based health care accrediting organization that establishes quality standards for the health care industry. The accreditation is valid for three years.

URAC’s standards keep pace with the rapid changes in the health care system, and provide a mark of distinction for health care organizations to demonstrate their commitment to quality and accountability. The Center underwent an examination of its organizational and case management systems, processes, and performance and was granted the Case Management accreditation because of its strong transition of care programs that utilize best practices to reduce hospital readmissions and improve health outcomes.
