Care at Hand Used in Hospital Readmission Reduction Program

In January 2015, The Coordinating Center began using Care at Hand, a mobile technology to capture and analyze observations for a hospital readmission reduction program. The Coordinating Center has been working with three West Baltimore hospitals and their fee for service Medicare beneficiaries—Bon Secours Hospital, University of Maryland Main Campus and University of Maryland Midtown Campus—to reduce the readmission rates of individuals with chronic illnesses.

Renée Dain Named Vice President of Business Development and Social Innovation at The Coordinating Center

The Coordinating Center is excited to announce that Renée Dain is the new Vice President of Business Development and Social Innovation.

Ms. Dain brings more than 12 years of experience in community outreach and developing new partnerships and initiatives. As Vice President of Business Development and Social Innovation, Ms. Dain will be part of the executive team that drives the overall strategy for The Coordinating Center and will represent The Center on a local, state and national level. She will explore and seek new business opportunities in the areas of social determinants in health, aging, disabilities, affordable housing, Medicaid and Medicare programs, hospital readmission reduction programs, population health and the Affordable Care Act.

The Coordinating Center’s Vice President, Quality and Outcomes Management Appointed to National Quality Forum Committee

Colby Bearch, MA-SF, MA-M, BA, RN, CDONA, Vice President, Quality and Outcomes Management at The Coordinating Center has been appointed to the National Quality Forum’s Care Coordination Steering Committee. The committee will review performance measures in care coordination, including those focused on patient experience of care, health information technology, transitions of care and structural measures.

The Coordinating Center Sets Ribbon Cutting and Open House for New Headquarters Building

The Coordinating Center will officially open its new Anne Arundel County headquarters on Thursday, November 14 with a Ribbon Cutting at 9:30 AM at the office, 8531 Veterans Highway in Millersville, Maryland.  The ceremony will be followed by an Open House and public reception until 11:00 AM. The Center is a non-profit organization serving families and individuals with complex needs and disabilities throughout Maryland.
